the Bark Bath

By completing my booking I do hereby agree to waive and release any and all claims, demands, and causes of action of any kind of nature whatsoever, including but not limited to personal injury, property damage, and death, against Bark Park Playcentre for Dogs, its owners, employees, agents, and successors in interest, arising out of or in connection with the grooming of my dog,

I understand that dog grooming can be a dangerous activity, and that there is always the risk of injury to both the dog and the groomer. I also understand that Bark Park Playcentre for Dogs will take all reasonable precautions to ensure the safety of my dog, but that accidents can happen.

My acceptance and payment of my booking, acknowledges that I agree to assume all risks associated with the grooming of my dog, and I agree to hold Bark Park Playcentre for Dogs harmless for any and all claims arising out of or in connection with such grooming.

Statement of Dog Owner Responsibility

I understand that it is my responsibility to disclose any and all known health problems or behavioural issues with my dog to Bark Park Playcentre for Dogs prior to grooming.

I understand that failure to disclose such information could result in injury to my dog, the groomer, or other animals present at the grooming salon. I also understand that Bark Park Playcentre for Dogs may refuse to groom my dog if they are not aware of any potential health or behavioural risks. I agree to hold Bark Park Playcentre for Dogs harmless for any and all claims arising out of or in connection with any injury or damage caused by my dog's undisclosed health problems or behavioural issues.

I agree to not bring my Dog for a Grooming appointment if they are displaying any signs of illness, have been diagnosed with a contagious illness or have been sick in the past 30days with a contagious illness. I declare that my Dog has up to date vaccinations. 

 

Conditions of Payment

I agree to pay a deposit at the time of booking and understand that this is non-refundable. I understand that it will come off my total payable and that there can be additional costs with the service. I understand that Bark Park Playcentre for Dogs reserves the right to adjust their prices without notice.

I understand that by booking an appointment on a public holiday I will incur a surcharge of 15% on the service booked.
 

My acceptance and payment of my bookings is inclusive of the waiver of general liability and acceptance of these terms and conditions.
